Registration

Anyone living within the Worksop NHS Boundary can register at Westwood 8 to 8 Primary Care Centre. Patients who wish to register should complete a registration form that can be collected from a member of the Reception team who will be able to guide you through the simple registration process.

All new patient registrations will be entitled to receive a health check. This is your opportunity to take advantage of an assessment of your health and wellbeing.

Westwood 8 to 8 Primary Care Centre are registered under the Data Protection Act and all records are stored on computer and paper is covered by our confidentiality procedure. At times you may need to be asked to complete a patient questionnaire in order to update and maintain our records. Please keep the practice up to date about any changes in name, address or telephone number.

You may be receiving care from other care providers as well as the NHS and we may need to share some information from time to time to help provide the right level of care for you. This would only be used or passed on if there was a genuine requirement for this. Wherever possible we will remove details which identify you and anyone who received this information from us has a legal duty to keep this confidential.
