Doncaster 8 - 8 Health Centre is managed by DMSL in conjunction with Hallcross Medical Services Ltd where a GP Led Equitible Access NHS Healthcare Service is provided.
We are open from 8am - 8pm, 7 days a week, 365 days a year (including bank holidays).
Our services don't just stop there, we offer so much more to our patients than convenient opening hours, where healthcare professionals are available 365 days a year. Our aim is to provide quality medical care, encourage good health for our patients, help our patients prevent disease and to also help patients to make informed decisions about their own health.
We are currently accepting new patient registrations within the NHS Doncaster Boundary. Just complete a simple registration form (we have translation services available should you require these, please let us know in advance if you would like to use this service.) All new patients are offered a comprehensive new patient health check.
Even if you are not registered with Doncaster 8-8 Health Centre you are still eligible to use the 8-8 health centre services.
Named GP For Aged 75 And Over
Unplanned admissions to hospital are distressing and disruptive for patients, carers and families. Many unplanned admissions are for patients are for patients who are elderly, infirm or have complex physical or mental health and care needs which put them at high risk of unplanned admission or re-admission to hospital.
Doncaster 8 to 8 Health Centre will be working closely with this group of patients by developing Care Plans for each patient, establishing a named GP for each patient to take the lead on any health issues and improving access to services at the practice.
Patient Access To Their GP Record
From April 2014 GP Practices are required to promote and offer the facility for patients to view online, export or print any summary information from their records ie. medications, allergies, adverse reactions and any additional information agreed between the GP and patient.
The plan for Doncaster 8 to 8 Health Centre is to review this after 1st April 2014 with a view to this information such as medication, allergies and adverse reactions being available to our patients by 31st March 2015.
GP Practices are now contractually required to include the NHS Number as the primary identifier in all NHS clinical correspondence issued by the Practice, whether in electronic form or otherwise, except in exceptional circumstances where the number cannot be ascertained eg. when a patient is seen urgently at home.
Doncaster 8 to 8 Health Centre already include the NHS Numbers as the primary identifier on all NHS clinical correspondence.
Electronic Appointment Booking
GP Practices are now contractually required to promote and offer the facility for patients to book, view, amend, cancel and print appointments online.
Doncaster 8 to 8 Health Centre offers all patients the facility to book, view, amend, cancel and print appointments online.
Online Booking of Prescriptions
GP Practices are contractually required to promote and offer the facility for patients who wish to order online, view and print a list of their repeat prescriptions for necessary drugs, medications and appliances.

Summary Care Record
GP Practices are required to provide an automated upload of any changes to a patients summary information, at least once every working day, to the Summary Care Record (SCR).

GP2GP Record Transfers
There is a contractual requirement to utilise the GP2GP facility for the transfer of patient records between practices, when a patient registers or de-registers (not for temporary registration)
Patient Participation Group
We would like our patient's feedback on our service, surgery and staff. This will enable us to assess any area of improvement that you, the patients, would like to see.
To help us with this, we have set up a virtual patient participation group (email: DONCCG.NationalParticipationGroup@nhs.net ) so that you can have your say. We will ask the members of this group some questions from time to time, such as what you think about our opening times or the quality of care or service you received and listen to what you have to say as a patient and what changes you would like to see. We will also ask for your comments and feedback on surveys that we would like our registered patients to complete. All contact will be via email.
We are keen for our registered patients to join our virtual Patient Participation Group. Should you wish to be part of this group, please either email DONCCG.NationalParticipationGroup@nhs.net to request a sign up form, or ask at reception.
